A 34 year old man presents to the emergency department with a one week history of malaise. In the past 24 hours he has also developed a marked rash all over his body.

The rash is painless, does not itch, and is widespread. He has taken paracetamol for general aches and pains, but little else.

He is normally fit and well, takes no medication and has no significant family history.

He is a single homosexual man. He is a non-smoker, drinks more than he should (30-35 units per week) and lives alone.
