Andrew Rhysm, a middle aged man is brought to the ED at 22:40 by ambulance. He is seen at 23:00 and noted to be complaining of pain in his upper back. He describes a pain that came on suddenly 3 hours earlier while watching TV. The pain is worse on movement and inspiration but does not radiate. He is not nauseated and has not vomited. He has been given 10mg morphine by the paramedics who brought him straight to hospital.

He had a past history of stroke and a cardiomyopathy, for which he is on warfarin.
