A young chap is brought to the paediatric ED following a fall in the park. there were some high jinks involved and the story is a bit vague (isn't it often), but it appears that he was sliding down a high slide on his feet, i.e. stood up.

This did not go well and at some point he seems to have gone over the side and landed on his legs.

He tells you that he had immediate and severe pain in his knee and is now unable to weight bear.

You have a look a that leg and see that he does indeed have quite a large effusion. It's all generally pretty sore so you get him some analgesia and wheel him round to radiology for some X-rays.
